Extensive pulmonary sarcoid reaction in a patient with BMPR-2 associated idiopathic pulmonary arterial hypertension.

چکیده
Pulmonary arterial hypertension is a progressive life-threatening disease characterized by vascular remodeling. There is evidence that varied immune mechanism play an important role in progression of pulmonary hypertension. We describe a case of a 35-year-old woman with idiopathic pulmonary arterial hypertension (IPAH) and a novel BMPR2 mutation, who underwent a successful lung transplantation. Extensive granulomatous inflammation was seen in the resected lungs. The granulomatous inflammation found in the histology supports a sarcoid-like reaction due to pulmonary hypertension in the context of the BMPR2 mutation.

Pulmonary Hypertension Registry: a 10-Year Experience
Background: Pulmonary Arterial Hypertension (PAH), a fatal orphan disease, is defined as increased mean Pulmonary Arterial Pressure (mPAP) above 25 mm/Hg, Pulmonary Capillary Wedge Pressure (PCWP) below 15 mm/Hg, and pulmonary vascular Resistance over 3 Wood Unit (WU). Although Pulmonary Hypertension (PH) is a relatively prevalent disease, the idiopathic form (iPAH) is a rare disease with a pre...
متن کاملBone morphogenetic protein signalling in heritable versus idiopathic pulmonary hypertension.
Mutations in the gene encoding bone morphogenetic protein (BMP) receptor type 2 (BMPR-2) have been reported in pulmonary arterial hypertension (PAH), but their functional relevance remains incompletely understood. BMP receptor expression was evaluated in human lungs and in cultured pulmonary artery smooth muscle cells (PASMCs) isolated from 19 idiopathic PAH patients and nine heritable PAH pati...

متن کاملA case of severe pulmonary hypertension associated with common arterial trunk and VSD in a 9 years old child
Background: Common arterial trunk (CAT) is a rare congenital heart disease, and often leads to the early development of pulmonary hypertension and disability. Among the critical structural heart defects, the frequency of CAT is 3%, which reflects the severe hemodynamic disturbances. The natural course of the disease is characterized by a high mortality rate up to 88% during the first year of li...

111 Dysfunctional bone morphogenetic protein (BMP) signaling has been found in patients with pulmonary arterial hypertension (PAH); however, the exact role of BMP signaling in the treatment of PAH remains unknown. The BMP receptor type II (BMPR-II) is a member of the TGF-β family of signaling molecules. Functional receptors are heterodimers composed of a BMPR-II subunit and a serine–threonine k...
